The Practice offers nurse led clinic appointments to all patients with a long term condition such as diabetes, cardiac problems or respiratory problems.
On-going monitoring of the condition, to prevent complications, is very important and these appointments are an opportunity for patients to learn about, and understand, their condition. Patients are also offered a personal management plan to assist in their care.
